Auto Substitutions
Auto Substitution occurs when an item ordered is not a primary drug, or is currently unavailable for order. In these situations, McKesson will send a primary or secondary generic interchangeable. This avoids ‘manufacturer cannot supply’ (MCS) situations.
There are four types of substitution:
- Different pack size is sent than ordered, same DIN.
- Primary generic is sent when secondary generic is ordered.
- Secondary generic is sent when primary generic is ordered but unavailable.
- Primary or secondary generic is sent when non-formulary generic is ordered.
Receiving Auto Substituted Drugs
The Auto-Reconcile Exceptions Report will print each time a purchase order is applied in Nexxsys.
- This report will list which drugs have been substituted.
- It also indicates which drugs were not automatically reconciled and therefore require attention.

The paper invoices sent with the order will also indicate which drugs were substituted, as well as which drugs were not shipped.
Note: It is important to fully investigate any drugs that aren’t received to ensure that any patients with owe balances or medications on order receive their prescriptions.
Bypassing Auto Substitution
It may be required to order a product that is auto-subbed by McKesson.
- If it is a regular drug:
- Call McKesson Customer Service for override.
- Provide agent with item number specific to the drug and pack size required.
- Provide reason for override (e.g. patient’s choice).
- If it is a narcotic drug:
- Add to order pad as usual. For more information, see Manually Adding a Drug to the Order Pad.
- Send order as usual. For more information, see Sending the Order.
- When at Narcotic Suggestion stage of submitting order in PharmaClik, you will be notified you are ordering a non-primary generic.
- In the Reason for Ordering column, select O.
